Abstract: O Câncer do Colo Útero (CCU) é uma das neoplasias mais frequentes entre as mulheres e a infecção persistente pelo Papilomavírus Humano (HPV) de alto risco oncogênico é necessária, mas não suficiente para o seu desenvolvimento. Apesar de estudos terem avaliado a associação do folato com o CCU, os resultados ainda não estão bem estabelecidos. Na maioria destes trabalhos foi avaliado o folato sérico e pouco se conhece sobre a relação entre os níveis de folato intracelular cervical e a presença da infecção pelo HPV e alterações pré-neoplásicas na cérvice uterina. Por isso, o objetivo deste estudo foi avaliar a associação dos níveis séricos e intracelulares cervicais de folato com a infecção pelo HPV e com as lesões cervicais. Foram selecionadas 177 mulheres, divididas em dois grupos: Controle (n=120), composto por mulheres com citologia normal, e Casos (n=57), divido em mulheres apresentando Células Escamosas Atípicas de Significado Indeterminado (ASC-US) (n=21), Lesão Intraepitelial Escamosa de Baixo Grau (LSIL) (n=16) e Lesão Cervical de Alto Grau (n=20), sendo esse constituído por mulheres apresentando Lesão Intraepitelial Escamosa de Alto Grau (HSIL) ou Células Escamosas Atípicas onde não se pode excluir lesão de alto grau (ASC-H). A detecção e a genotipagem do HPV foram realizadas por PCR-RFLP e as concentrações séricas e intracelulares cervicais de folato foram determinadas por imunoquimioluminescência. O valor mediano de folato sérico entre as participantes foi de 11,9 ng/mL, variando de 4,0 a 42,6 ng/mL. Por outro lado, a mediana de folato intracelular cervical foi 2,5 ng/mg (0,4−26,6 ng/mg). As concentrações séricas e intracelulares cervicais de folato não diferiram entre as participantes apresentando ou não infecção pelo HPV (p=0,344 e p=0,502, respectivamente). Mulheres com alterações citopatológicas de alto grau (HSIL+ASC-H) apresentaram menores concentrações do micronutriente, sérico (10,8 ng/mL) ou intracelular cervical (2,0 ng/mg), se comparado aos demais grupos (controle, ASC-US e LSIL), embora esta diferença não tenha sido significativa (Folato sérico: p=0,610; Folato intracelular cervical: p=0,817). Assim, não foi observada associação dos níveis séricos e intracelulares cervicais de folato com a infecção pelo HPV ou lesões cervicais.

In majority of these studies serum folate was analyzed and little is known about the relationship between cervical cellular folate levels and presence of HPV infection and pre-neoplastic abnormalities in uterine cervix. Therefore, the objective of this study was to evaluate the association of serum and cellular cervical folate levels with HPV infection and cervical lesions. 177 women were selected and divided into two groups: Control (n=120), with women presenting normal cytology, and Cases (n=57), that was composed by women presenting Atypical Squamous Cells of Undetermined Significance (ASC-US) (n=21), Intraepithelial Lesion (LSIL) (n=16) and High-grade lesions (n=20), with women presenting High Grade Squamous Intraepithelial Lesion (HSIL) or Atypical Squamous Cells where high grade lesion cannot be excluded (ASC-H). HPV detection and genotyping were performed by PCR-RFLP, and cervical serum and intracellular folate concentrations were determined by immunochemiluminescence. The median serum folate among participants was 11.9 ng/mL, ranging from 4.0 to 42.6 ng/mL. On the other hand, median intracellular cervical folate was 2.5 ng/mg (0.4−26.6 ng/mg). Serum and intracellular folate concentrations did not differ among participants with or without HPV infection (p=0,344 e p=0,502, respectively). Participants with high-grade cervical lesions (HSIL+ASC-H) presented lower concentrations of serum folate (10.8 ng/mL) or cervical cell folate (2.0 ng/mg), if compared to other groups (Control, ASC-US or LSIL), although this difference was not significant (serum folate: p=0.610, cervical cellular folate: p=0.817). Therefore, there was no association of serum and cervical cell folate concentrations with HPV infection or pre-neoplastic lesions in uterine cervix.
